This is an online form where you will answer questions about your situation. The information you give will help us to work out what type of support you may need. We will ask you questions about your care and support needs.
You can complete the assessment on behalf of somebody else, such as a friend or relative. You will need to have their consent to do this. You should get them to help answer the questions.
Request care and support online. Request a professional referral. If you are a professional and are concerned about a young person under 18 years old please complete a safeguarding children universal referral form. This is a conversation where a member of our care and support team will ask you about your situation. This will help us to understand your needs and work out what type of support you may need.
