Local counties contract with HDC to provide case management services to adults and children with serious mental health issues. Case managers work closely with individuals and families to help them access needed services and support to maximize independence, mental health, and community integration. Available Locations: Carlton , Lake , and St.
Louis Counties. Psychotherapy is provided in individual, family, and group settings. Some issues addressed by HDC therapists include depression, anxiety, grief, abuse, life stress, family and marital issues, pain management, eating disorders, and anger management. Services are open to children, adolescents, and adults. Psychiatry services through HDC include evaluations, treatment, and medication management for depression, anxiety, and other psychiatric disorders. HDC psychiatrists and psychiatric nurses are skilled in pharmacology for children and adults.
Services are offered in HDC offices and at contracted locations. Rehabilitative services are available through ARMHS and help individuals acquire skills to live independently in the community. Services are provided to people who have a mental illness or difficulty in their ability to live independently and meet the medical necessity criteria for service.
BHH services are an innovative, integrated healthcare service delivery model that is recovery-oriented and person- and family-centered. BHH allows for better individual experiences and better outcomes than those achieved in traditional services.
A BHH team is comprised of a registered nurse, systems navigators, and qualified health home specialists. Coordination with behavioral health, primary care, family practice, pediatric, and all healthcare providers is key to services in the BHH model.
